NOTE:
- *The NSITF number must be stated on the sworn affidavit.
- *The sworn affidavit and indemnity letter must be duly stamped by the High Court.
- *The Indemnity Letter can be classified into two different type i.e. Existing Employer and Non-existing employer. For customer whose employer is non-existing, such customer is to indemnify himself or herself, while for customer whose employer exists, the employer is to indemnify the customer and place the indemnity letter format on the company’s letter-head.
- *Any form of name discrepancy such as OMONIYI and NIYI, should be backed with a name confirmation.
- *For change of name, the customer should provide us with newspaper publication or marriage certificate.

Where not all administrators on an LOA are signatories to the account of the deceased, Power of Attorney duly certified by the probate registry MUST be provided by those administrator(s) who wants to transfer their powers to the signatory (ies):
